Submit follow-up information

Add new information, or send medical records, for a report that has already been submitted.

When to send follow-up information

Send follow-up information when something about the report has changed or was not known at the time, for example:

  • The outcome changed, including recovery, admission or death.
  • A diagnosis was made after the report was submitted.
  • You now have discharge summaries, notes or test results.
  • A detail in the report was wrong, such as a lot number.
  • VAERS staff wrote to you asking for records.

Finding your VAERS ID

The VAERS ID is on the confirmation shown when the report was submitted, and in the confirmation email. Follow-up cannot be matched to a report without it. If you do not have it, contact VAERS and give the patient details and the approximate date of the report.

Sending medical records

For reports classed as serious, VAERS requests medical records so the event can be reviewed properly. Records can be sent by secure upload, by fax or by post. Do not send records by ordinary email.
