Resume a saved report

Enter the resume code you were given when you saved your report. This opens the report where you left off.

How resume codes work

Saving a report gives you a six-character code, for example ACDE-F3. The code opens the draft again where you left off. Nothing is sent to CDC or FDA until you submit the report.

  • The code is also shown at the top of the form while you work.
  • No account and no email address are needed.

If the code does not work

Codes do not use the letters I, L, O, S, B or Z, so a character that looks like one of those is likely a 1, 0, 5, 8 or 2. If you still cannot open the report, start a new one — nothing was submitted.
